Mary Ann Chaklos, Administrator
Mary Ann’s background represents more than 30 years as an Administrator working for both large and smaller skilled nursing homes across Central Pennsylvania. Most recently Mary Ann was blessed to be associated with the Sisters of St. Cyril and Methodius in a non-profit Continuing Care Retirement Community. Mary Ann’s values are deeply rooted in her faith, community and passion for the senior community. Mary Ann is guided by transparency and accountability; you will find that Mary Ann is highly committed to those entrusted to my care. The most rewarding part of her career is getting to know our residents on a personal level.

Michelle Burton, Director of Rehabilitation
Education: M.S. Occupational Therapy; College Misericordia
Professional Certifications: Occupational Therapist, Registered, Licensed
Michelle provides leadership and direction to the daily operations of the rehabilitation department, and facilitates the development of rehabilitation plans of care for residents in the subacute care unit. She also assists in maintaining professional standards of practice in the rehabilitation department. She has 19 years of experience as an occupational therapist, and she enjoys “the daily interactions with the dynamic and diverse patient population at Spring Creek,” as well as “working together with a strong team of professionals to serve our residents.”
Nathan Mitchell, Director of Activities
Education: A.A. in Visual Art and Science; Harrisburg Area Community College, B.A. in Visual Art and Science; Shippensburg University
Professional Certifications: National Council of Certified Dementia Practitioners, National Certification Council for Activity Practitioners
Nathan oversees activities for both short-term and long-term residents. He enjoys engaging with residents through meaningful and productive programs, and firmly believes that spending time getting to know the residents makes a meaningful difference in their lives. He also appreciates the fact that each resident has a unique personality, different backgrounds, and diverse stories to share. At Spring Creek, he leads a team that is deeply engaged in the lives of their residents, and is constantly implementing creative new ideas. Nathan says that his “passion in life is to make sure people know that they matter and help them find something that makes them happy,” and adds that being an activities professional is one of his proudest accomplishments
“The reason I’ve stayed for 30 years is because it’s warm, it’s welcoming, you meet new people. I like being around the residents. We talk a lot, try to make each other laugh, which I love.”
